• valid identity card or passport
• for commercially used vehicles: appropriate proof
• in the event of loss: declaration of loss with affidavit
• in the event of theft: confirmation from the police of the theft report
• existing registration certificate part II (formerly: vehicle registration document)
• Test report on a valid general inspection in accordance with the Road Traffic Licensing Regulations (StVZO)

if applicable:

• if represented by another person: written power of attorney and valid identity card or passport of the authorized person
• for minors: Declaration of consent and identification documents of the legal guardian
• Written formal application to the responsible registration authority with details of the owner and vehicle data
  The application can also be completed at the registration authority.
• Valid identity card (not a copy) of the vehicle owner or passport with registration certificate from the registration office of the place of residence
  The registration certificate must not be older than three months.
  If the vehicle owner is not a natural person, further documents are required to identify the owner.

Additionally in the event of damage:

• The damaged registration certificate must be presented to the registration authority. A replacement will be issued.

Additionally in the event of loss:

• the remaining Part I or II of the registration certificate (vehicle registration document or vehicle registration certificate)
• Affirmation in lieu of an oath (a telephone or written notification to the registration authority is not sufficient); declaration of loss, if applicable
  In the event of theft, confirmation of the police report.
• Inspection report of the last general inspection that has not yet expired (not applicable if the first general inspection was not yet due)
• Inspection report of the last exhaust emission test that has not yet expired (not applicable if the first exhaust emission test was not yet due and for vehicles that are not subject to mandatory exhaust emission testing)
• Inspection report of the last safety inspection that has not yet expired (if required)
