You will need the following documents:

• Curriculum vitae in German with a tabular presentation of your completed training courses and gainful employment,
• Information about your current place of residence,
• Proof of identity, proof of citizenship,
• Evidence of education showing the qualification acquired abroad to work in the public service in the field of medical and health sciences,
• Evidence of the content and duration of studies and training in the form of study regulations, examination regulations, study book or in any other appropriate manner; the evidence must indicate the requirements that led to obtaining the degree,
• certificate issued by the Member State stating the profession to which the professional qualifications entitle the holder there,
• certificates of the type and duration of the activity you have performed in a Member State in the public service in the field of the qualification after obtaining it,
• Declaration of what professional activity you intend to pursue in the public administration on the basis of the qualification certificate,
• Declaration whether and to which body you have already submitted an application for the determination of equivalence, if applicable, decision on the determination of equivalence,
• certificates or documents from the Member State of origin confirming that you are not aware of any criminal offences, serious professional misconduct or other circumstances that might call your suitability into question; the certificates or documents must not be older than three months at the time they are presented.

Depending on the profession, further documents may be required. Please contact the competent authority. As a rule, you must submit all documents in the original or as a certified copy. As a rule, you must submit all documents in German. If the documents are written in another language, you may also need a translation by a publicly appointed translator.
